To join the tournament, first make a PayPal donation, then contact us with your name and email address and we'll send you all the relevant information! The deadline to sign up is April 30th.

To sign up and/or donate by mail: Download and print the entry form (PDF) Make checks payable to: Mail checks to: Checks should be made out for $100 for each golfer playing. For example, if you are signing up three people, the check or checks should total $300. If you mail a check in, be sure to contact us using the form to the left to let us know you are playing! All donations are tax deductible! |
